I undertake to accept responsibility for the instrument hired from Ruyton Girls’ School, for use by my daughter. *
I understand that the hire fee for this instrument is $100 per term ($200 per semester) and that this amount will be payable through Clipboard. *
I understand that the school will cover the cost of general maintenance, however, I accept responsibility for the cost of replacement strings, rosin, etc. and any damage to the instrument whilst in my daughter’s care. In the event of loss or damage beyond repair, I agree to pay the replacement cost OR Ruyton’s Insurance Policy Excess ($1,000), whichever is the least.

I also undertake to report any theft, loss or damage immediately to the Performing Arts Office. *
I have read the conditions of enrolment and agree to give 4 weeks notice (excluding school holidays) for cessation of lessons by filling out the Discontinuation Form available on the school intranet. *
